catgate
From a news item on Google news:-Another waste of money - Ofqual, the Government body set up to monitor national tests in schools, paid consultants nearly £3,000 to come up with another word to use when talking about mistakes in examinations. The right word - error - was considered not suitable as it implied blame and had 'negative impacts'! Instead, we are told that 'variation' is more acceptable.

AlanHo
Their decision is quite clearly a variation. A more suitable substitute for "error" might have been "total balls-up"
