I'm running Windows XP pro at work and yesterday when I logged on I noticed some programs, files, and settings were missing from my computer. It seems they were all in my user profile directory. The Google Chrome executable (my settings were saved), some random files on my desktop, iTunes settings, and Virtual Box settings were gone (when these ran it was like this was the first time).

I restored my settings the best I could but sometime today I noticed again Google Chrome was gone and my iTunes and Virtual Box settings were gone.

I've run two spyware scanners and virus scan with nothing detected.

I've run chkdsk c: /f already but when I check inside Windows I'm getting this output:

CHKDSK is verifying files (stage 1 of 3)...

File verification completed.

CHKDSK is verifying indexes (stage 2 of 3)...

Index verification completed.

CHKDSK is verifying security descriptors (stage 3 of 3)...

Security descriptor verification completed.

CHKDSK is verifying Usn Journal...

Usn Journal verification completed.

CHKDSK discovered free space marked as allocated in the

master file table (MFT) bitmap.

Correcting errors in the Volume Bitmap.

Windows found problems with the file system.

Run CHKDSK with the /F (fix) option to correct these.

What could be going on here?

After I rebooted, I immediately ran chkdsk again and got this output:

CHKDSK is verifying files (stage 1 of 3)...

File verification completed.

CHKDSK is verifying indexes (stage 2 of 3)...

Deleting index entry MpSfc.bin in index $I30 of file 14825.

Deleting index entry RU64EC~1.PF in index $I30 of file 22245.

Deleting index entry RUNDLL32.EXE-4C9875AB.pf in index $I30 of file 22245.

Index verification completed.

Errors found. CHKDSK cannot continue in read-only mode.

I'm thinking the disk could be failing.

Most likely, I would say. Try to back up all your data, then call in a technician to test the hard disk.
